4.) Information reasonably sufficient to authorize us to contact the complaining party, such as a location, telephone number, and, if possible, an electronic mail address at which the objecting party may be communicated.
5.) A report that the objecting party has a good faith belief that use of the material in practice accused of is not authorized by the copyright owner, its operator, or the law.
6.) A report that the information in the warning is accurate, and under penalty of perjury. That the objecting party is authorized to act on behalf of the owner of an exclusive right allegedly violated.
